A apneia obstrutiva do sono é um distúrbio da respiração durante o sono, associado a uma alta taxa de morbidade e mortalidade. Vários fatores anátomo-estruturais e
neuromusculares que ocluem a faringe de forma intermitente são considerados para o desenvolvimento da síndrome da apneia-hipopneia obstrutiva do sono (SAHOS). Os tratamentos cirúrgicos englobam as cirurgias de tecido mole, do nervo estimulador do hipoglosso, e cirurgias ortognáticas. A cirurgia ortognática bimaxilar, devolve qualidade de
vida e melhora em vários sintomas. A genioplastia, outro tipo de cirurgia ortognática, tem se mostrado efetiva, reduzindo o índice de apneia hipopneia em 45.7%. O objetivo do presente estudo foi o de revisar a literatura sobre a abordagem e manejo da SAHOS pela cirurgia bucomaxilofacial como foco no efeito da genioplastia em pacientes portadores da SAHOS. Sendo assim, buscou-se resposta para a pergunta da pesquisa: a cirurgia ortognática do avanço do mento pode melhorar o índice de Apneia-Hipopneia (IAH) ou porcentagem de saturação de oxigênio de pacientes com SAHOS? As bases de dados pesquisadas foram Pubmed e Google Acadêmico, resultando em uma revisão bibliográfica. Um total de doze estudos foram encontrados, referindo melhoras nos pacientes operados pela genioplastia. A genioplastia mostrou-se eficaz no manejo cirúrgico para os pacientes com SAHOS.

Abstract
Obstructive sleep apnea is a sleep-breathing disorder associated with a high morbidity and mortality rate. Several anatomical and neuromuscular factors have been linked to obstructive sleep apnea-hypopnea syndrome (OSAHS). Surgical treatments include soft tissue surgery, hypoglossal stimulating nerve, and orthognathic surgery. Bimaxillary orthognathic surgery improves both symptoms and quality of life. Genioplasty, a less invasive type of orthognathic surgery, has been shown as effective in reducing the apnea-hypopnea index by 45.7%. The present study aims to review the literature about the management of OSAHS through maxillofacial surgery, focusing on the effect of genioplasty in patients with OSAHS. Therefore, we sought to answer the research question: Can genioplasty improve OSAHS patients' Apnea-Hypopnea Index (AHI) or oxygen saturation(sat.O2)? Pubmed and ScholarGoogle databases were searched. Twelve studies were found reporting improvements in patients undergoing genioplasty. Genioplasty showed to be effective in surgical management for patients with OSAHS.
